During root canal treatment it is possible to break an instrument into the canal.
This may be due to one or more of the following reasons:
- Anatomy of the tooth – narrow curves in which the instrument might bend and break.
- Warn out material of the endodontic instruments.
- Lack of sufficient experience and knowledge of the working dentist.
This begs the question of whether or not to remove the broken instrument.
If the tooth does not have an apical lesion (inflammation around the tip of the root) and a dental dam (kofferdam) was used then we have the necessary disinfection. In this situation, if the instrument breaks at the end of treatment in a complex curvature of the canal, we may as well leave it. This tooth should be monitored with X-rays for the next five years.
However, when an X-ray shows inflammation around the root (granuloma) and symptoms (pain, edema, etc.) are present, the fragment of the broken instrument must be removed to process and disinfect the canal and complete the treatment of the tooth successfully.

What it takes to successfully remove a separated instrument:
- Microscope – you can’t remove something you can’t see.
- Tooth insulation with kofferdam.
- Appropriate instruments for this purpose. Many methods and systems are described, but the specialist chooses the one that works in his or her hands the best.
- Dentist qualification and sufficient experience.
